We are so grateful you have trusted us with your care. We have listed some procedures and information related to new patient scheduling and processing in our office.

  • We do require a referral to better care for you and understand your needs.

  • Once we receive your referral, our Nurse Practitioners will review the information.

  • The referral is then is placed with our referral coordinator for scheduling.

  • We call each patient at least two times to attempt scheduling

    • We schedule a “NEW PATIENT” visit along with a “SECOND VISIT”. The second visit is generally scheduled 2 weeks out from the new patient visit, give or take.

    • The two weeks in between the appointments provides time for lab testing to be completed and processed.

  • We then send out a new patient packet to be completed prior to the new patient visit, this can be sent out one of two ways:

    • Electronically

    • Via mail

      • We assure you that while it may seem like a lot of paperwork and personal questions, every question we ask is for the benefit of the patient and assists us in being as thorough as possible.

    • NEW PATIENT visit

      • This is when you will meet your provider and the rest of our office staff. We will discuss history of several types, recent symptoms, recent visits with other providers, previous labs and workup that had been done. We will discuss lifestyle and anything else that may be pertinent depending on each individuals needs.

        • We then order the workup appropriate for the individual patient

    • SECOND VISIT

      • This is to ensure that any labs or imaging that we ordered to aid us in timely and accurate diagnosis is discussed quickly with each patient. We believe each patient has the right to discuss such an extensive workup one on one with their provider so everything is understood. We print a copy of each patients lab and imaging and discuss them each one by one.

      • We diagnosis and educate during this visit if appropriate.

    • We pride ourselves in our thorough workup for new patients. We tend to have a different comprehensive approach as we feel that there are several things that can cause pain and chronic illness and it is our responsibility to do our due diligence to find our patients answers that can help them. With this being said, our workup is extensive yet customizable to fit each individuals needs.

  • If you have any questions at all or are having any issues completing the paperwork, please do not hesitate to call our office. Our referral coordinator can be reached at
